Requirements
- Have a bachelor's degree and a Master of Social Work (MSW) or MSW-equivalent degree.
- Hold an active Licensed Clinical Social Work (LCSW) license.
- Have at least two years of clinical work experience.
- Have experience using CBT or similar techniques.
- Have a working knowledge of medical terminology.
- Be comfortable using Slack, Zoom, Microsoft Office, and EHR systems.
- Hold Basic Life Support (BLS) certification by the role's start date.
- Comply with Daymark's Employee Vaccination and Testing policy.
- Cancer-care experience is preferred.
Responsibilities
- Provide CBT and regular individual sessions to address the mental health needs of patients diagnosed with cancer.
- Conduct initial needs assessments covering clinical or medication issues, behavioral concerns, safety risks, and psychosocial well-being.
- Develop care plans to address gaps in services, including palliative care, advance care, site-of-service shifts, and end-of-life planning.
- Connect members with local community services and public benefits.
- Develop, review, update, and maintain the community resource database and care protocols.
- Document patient and caregiver interactions in the EHR system.
- Stay in contact with caregivers to monitor side effects and changes in patient function or behavior.
- Consult with licensed clinical team members and triage complex or medical issues as needed.
- Participate in weekly patient rounds with the interdisciplinary care team.
