Requirements
- Have 2+ years of clinical experience serving patients with complex social and healthcare needs.
- Hold board certification in Family Medicine.
- Hold a medical license in California and additional state licensure in at least two of PA, TX, MD, MA, FL, NY, or NV; be willing to obtain additional licensure.
- Be eligible to enroll in Medicare.
- Hold an active California DEA or be willing to obtain one.
- Be physically located in California and have a quiet, HIPAA-compliant space with internet access.
- Be available for PST hours for urgent needs and support.
- Be comfortable caring for patients across the lifespan, from pediatrics to geriatrics.
- Have experience working with and in Medicaid/FQHC clinics serving underserved populations.
- Have experience working with Medicare beneficiaries managing chronic disease.
- Be experienced with digital-first healthcare such as telemedicine and comfortable using multiple technology platforms.
- Be available to work 5–10 hours per week to start.
Responsibilities
- Provide virtual care to Medicare patients with chronic, complex, and overlapping medical, behavioral health, and social needs.
- Supervise Nurse Practitioners providing multidisciplinary care to patients with serious mental illness, substance use disorder, homelessness, or high medical needs.
- Collaborate with physicians, Nurse Practitioners, care managers, and other care team members to support care coordination.
- Help develop and refine clinical programs to address patients’ holistic needs and care barriers.
- Use internal care delivery technology and clinical judgment to support virtual care for underserved patients.
