Requirements
- Have a high school diploma.
- Demonstrate strong organizational skills and proven follow-up ability.
- Have basic knowledge of medical terminology.
- Show attention to detail and strong written and verbal communication skills.
- Have a thorough understanding of healthcare technologies, including electronic health record systems.
- Be familiar with Microsoft Outlook, Microsoft Excel, and internet- or cloud-based applications.
- Communicate effectively and professionally with facility staff, colleagues, and medical team members.
- Have a quiet, distraction-free workspace and internet access at home.
- Be able to work EST hours.
Responsibilities
- Gather patient medical information between visits using electronic health record systems.
- Report monthly patient concerns, including falls, medication issues, behavioral concerns, and recent hospitalizations.
- Complete assigned principal care management interventions each month for approximately 15 patients per day.
- Report applicable issues to the ancillary program manager and/or Chief Clinical Officer.
- Provide additional employee training and shadowing support to newer employees.
- Track and monitor patient data and communicate across members of the medical team.
- Engage with patients and family members and report clinical summaries.
