Requirements
- Hold an active AHIMA Certified Coding Specialist (CCS) or AAPC Certified Inpatient Coder (CIC) certification.
- Have hands-on experience in inpatient medical coding and clinical documentation review.
- Understand ICD-10-CM and ICD-10-PCS coding systems.
- Demonstrate proficiency in MS-DRG methodology, diagnosis sequencing, and principal diagnosis selection.
- Know POA indicators, CCs, MCCs, UHDDS definitions, and applicable inpatient coding guidelines.
- Interpret complex clinical documentation and translate medical information into appropriate codes.
- Be familiar with AHA Coding Clinic guidance and current official coding standards.
- Work independently, follow detailed instructions, and deliver accurate coding annotations.
- Have access to current ICD-10-CM and ICD-10-PCS code sets, official coding guidelines, and appropriate MS-DRG grouper tools.
Responsibilities
- Review inpatient medical records, including admission notes, operative reports, test results, and discharge summaries.
- Assign diagnosis and procedure codes using ICD-10-CM and ICD-10-PCS.
- Identify principal and secondary diagnoses and apply appropriate sequencing rules.
- Review and validate MS-DRG assignments, including CC and MCC identification.
- Assign and validate Present on Admission indicators.
- Apply inpatient coding conventions, official guidelines, UHDDS definitions, and relevant AHA Coding Clinic guidance.
- Identify hospital-acquired conditions, patient safety indicators, and other coding-relevant clinical details where applicable.
- Annotate and validate clinical data for AI training and model evaluation.
- Investigate ambiguous documentation and provide evidence-based recommendations for complex cases.
- Provide structured quality feedback and collaborate with project stakeholders to clarify requirements and resolve documentation discrepancies.
