Requirements
- Hold an associate degree in nursing.
- Hold a Registered Nurse license in the state of Wisconsin in good standing.
- Care Management experience is preferred.
- Reside within 45 minutes or less of the assigned coverage area is preferred.
- Have a valid state driver's license.
- Have proof of personal vehicle liability insurance with at least 100/300/100 limits.
- Meet the work-at-home internet requirements: at least 25 Mbps download and 10 Mbps upload.
- Work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information.
- Be available Monday through Friday, 8:00 am–4:30 pm CST.
- Travel up to 40% for member visits.
Responsibilities
- Assess members’ health and safety needs to develop a comprehensive Member Care Plan.
- Conduct regular health assessments.
- Coordinate with acute and primary care providers.
- Educate members on disease processes, related risks, nutrition, exercise, and lifestyle modifications.
- Document in member files within required DHS contract timeframes.
- Participate in monthly phone contacts with members.
- Travel to attend in-person visits in members’ settings.
