Requirements
- Have direct leadership experience in transitions of care, post-acute, or care management operations.
- Have scaled or materially improved a post-discharge program.
- Know post-acute interventions.
- Have experience with clinical team training.
- Have experience with performance measurement for health plan populations.
- Be comfortable building workflows with clinicians and cross-functional partners in a growth-stage environment.
Responsibilities
- Lead and improve the 30-day transition pathway from admission and discharge alerts through visits, escalation, and follow-up.
- Scale medication reconciliation, DME, home health, PCP and specialist scheduling, transportation, and caregiver support.
- Coach distributed NPs, RNs, and MAs.
- Set protocols, documentation, quality checks, and reporting.
- Partner with product, engineering, and health plans to improve readmissions, PCR and TRC/HEDIS Stars measures, and program economics.
