- Prior experience coordinating or navigating care for patients, caregivers, and their loved ones.

Responsibilities
- Answer inbound calls, emails, and text messages from patients, caregivers, families, providers, and facilities.
- Schedule appointments and maintain contact with patients by phone or email.
- Explain health insurance benefits and eligibility, and process copays and coinsurance as needed.
- Ask screening questions under clinical supervision and document patient safety, behavior, function, medications, care needs, and advanced care plans.
- Screen and document caregiver issues, including burden, depression, and poor coping, under clinical supervision.
- Report screening results to the clinical team and coordinate care-plan implementation with patients, caregivers, providers, and community service providers.
- Send educational materials and information about Isaac Health and local community-based services as indicated in the care plan.
- Follow up after appointments, provide next steps, and coordinate care as documented by the provider.
- Request external medical records, lab results, and imaging results; document patient care and instructions in the EHR.
- Provide emotional support, answer questions within scope of practice, escalate patient concerns, and maintain privacy according to HIPAA regulations.
