Requirements
- Board certification or eligibility as a Family Physician.
- Medical license in California (willingness to obtain additional licenses).
- Active CA DEA or willingness to obtain one.
- 2+ years of clinical experience serving patients with complex social and healthcare needs.
- Experience with multidisciplinary team collaboration.
- Tech-savvy and experienced in digital-first healthcare or telemedicine.
- Physically located in California with a quiet, HIPAA compliant space.
- Eligible to enroll in Medicare.
- Experience working in Medicaid/FQHC clinics.
- Experience working with Medicare beneficiaries managing chronic disease.
Responsibilities
- Provide clinical supervision for Nurse Practitioners delivering multidisciplinary care.
- Deliver virtual care to Medicare patients with chronic, complex medical, and behavioral health needs.
- Collaborate with an interdisciplinary team of physicians, NPs, and care managers to address care gaps.
- Contribute to the development and refinement of clinical programs.
- Apply clinical judgment to support holistic patient care and address social determinants of health.
