Requirements
- Master of Social Work (MSW) degree.
- Current license to practice as a Master Social Worker (LMSW) or Clinical Social Worker (LCSW) in New York State.
- 1-3 years of relevant professional experience.
Responsibilities
- Provide culturally competent direct service to patients and their families using a variety of social work modalities.
- Utilize high risk criteria and consultation referrals to identify patients in need of assessment and intervention.
- Advocate on behalf of individuals to ensure maximum access to health care and assess barriers.
- Provide interdisciplinary team members with assessment findings and impact analysis.
- Deliver psychoeducation, counseling, and crisis intervention.
- Formulate and implement safe, timely treatment and discharge plans.
- Incorporate knowledge of community resources and entitlement programs into planning.
- Maintain professional patient care documentation according to site policies.
- Supervise undergraduate and graduate level Social Work students.
