Requirements

  • Associate's/Technical Degree or equivalent combination of education and related experience.
  • Three years of clinical experience.
  • Two years of clinical documentation experience.
  • Current licensed RN, medical provider, or equivalent.
  • Certified Clinical Documentation Specialist (CCDS) or Certified Documentation Improvement Practitioner (CDIP) certification.
  • Ability to interpret medical records and clinical documentation.
  • Knowledge of coding requirements and documentation guidelines.
  • Commitment to receiving all required vaccinations.

Responsibilities

  • Evaluate and assess medical records for clinical specificity, documentation accuracy, and coding requirements.
  • Perform coding, DRG assignment, and track review activity in software.
  • Analyze records to formulate and follow up on physician queries.
  • Update working DRG based on physician feedback or documentation updates.
  • Communicate documentation deficiencies or conflicting data to department leadership.
  • Act as a liaison between medical staff and the coding department to ensure accurate diagnosis capture.
  • Monitor regulatory changes and attend ongoing education sessions.
  • Attend physician and care management meetings to review assigned cases.
