Requirements
- RN license in good standing with willingness to obtain additional state licensures.
- Minimum of five years of RN experience.
- Fluent in English and Spanish (native-level reading, writing, and speaking).
- Willingness to work two late shifts (11 am - 8 pm PST) and three day shifts (8 am - 5 pm PST) per week.
- Ability and willingness to travel occasionally for corporate gatherings and events.
- Proficient computer skills, including Microsoft products.
- Ability to attend and engage professionally in video meetings.
- Reliable internet and designated workspace.
- Experience developing care plans for chronic conditions preferred.
- BSN highly preferred.
- CDE or CDCES certification highly preferred.
Responsibilities
- Establish and maintain contact with members via video, phone, text, and email in both English and Spanish.
- Develop individualized member and clinician-centered care plans per NCQA guidelines.
- Assess member health status and care coordination needs.
- Collaborate with member healthcare providers as needed.
- Utilize motivational interviewing and the Triple Aim framework to empower members.
- Explain program benefits to new or inactive members.
- Document and manage member information in accordance with HIPAA compliance.
- Meet individual performance metrics for member engagement and retention.
