- At least 2+ years of inpatient hospital experience preferred
Requirements
- B.S. in Pharmacy (PharmD preferred).
- Active and in good standing pharmacist license in Pennsylvania (PA) and/or Ohio (OH).
- At least 2+ years of inpatient hospital experience preferred.
- Overnight inpatient hospital experience strongly preferred.
- Knowledge of formulary management and P&T procedures.
- Demonstrated ability to use multiple pharmacy information systems and client/server applications.
- Proficiency in MS Office, Google Drive, and email tools.
- Reliable cable broadband or fiber optic internet (minimum 10Mbps download/5Mbps upload).
- Strong written and verbal communication skills.
- Demonstrated customer service skills.
Responsibilities
- Perform remote order entry, order verification, drug verification, and patient/staff counseling.
- Support multiple hospital clients while operating within various pharmacy information systems (PIS).
- Provide proactive clinical pharmacy services and therapeutic recommendations including dosing adjustments and infusion guidance.
- Investigate and resolve medication variances within 48 hours.
- Maintain working knowledge of IV fluids, TPN, narcotics, and chemotherapy.
- Document all orders, interventions, and recommendations in the system by the end of each shift.
- Participate in ongoing training, clinical updates, and policy reviews.
