Requirements

Board Certified as a Family Nurse Practitioner.
Active Family Nurse Practitioner license in KY, MI, OH, or IL.
Experience performing wellness visits.
Technologically savvy with proficiency in laptops, mobile devices, and HIPAA-secure messaging apps.
Strong written and verbal communication skills.
Ability to work both independently and collaboratively in a team environment.
High level of professionalism, integrity, and accountability.
Willingness to engage in proactive, last-minute problem solving.
Ability to pass a national background check and clinical license verification.

Responsibilities

Review daily patient caseload and prepare necessary technological tools.
Navigate daily schedules using mobile technology to partner with in-home clinical Sprinters.
Conduct virtual wellness exams, health risk assessments, medication reviews, and cognitive screenings.
Empower patients with educational information regarding health and well-being.
Collaborate with clinical teams to evaluate vitals, arrange blood draws, and address specific patient needs.
Provide comprehensive care coordination, including preventive interventions, specialist referrals, and community resource management.
