Experience: Minimum 5 years

Requirements

Licensed Registered Dietitian Nutritionist (RDN).
Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (BC-ADM), or eligible for certification.
Minimum 5 years of experience in direct patient care, preferably in a clinical or telehealth setting.
Strong knowledge of chronic cardiometabolic conditions including diabetes, hypertension, hyperlipidemia, and obesity.
Experience in medication adjustment.
Proficiency using Electronic Medical Records (EMR).
Experience providing care through virtual platforms including chat, phone, and video.
Ability to work in an interdisciplinary team environment.
Must be able to work 2 weekends per quarter.
A quiet, dedicated workspace free from distractions.
Reliable high-speed internet connection.

Responsibilities

Provide comprehensive diabetes, hypertension, hyperlipidemia, and weight loss care, education, and support for members through chat, phone, and video calls.
Create thorough consults aligned with internal protocols for physicians and the provider team.
Collaborate with Patient Care Advocates to ensure member success.
Develop tailored nutrition and lifestyle plans based on medical conditions and evidence-based guidelines.
Collaborate with pharmacists to provide guidance on chronic condition medications.
Track member progress, assess intervention effectiveness, and adjust plans based on clinical outcomes.
Provide motivational interviewing and counseling to support sustainable behavior changes.
Utilize member health metrics to help patients interpret lab results and track outcomes.
Maintain accurate member documentation in accordance with legal and internal standards.
