Requirements

  • Licensed Registered Dietitian Nutritionist (RDN).
  • Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (BC-ADM), or eligible for certification.
  • Minimum 5 years of experience in direct patient care, preferably in a clinical or telehealth setting.
  • Strong knowledge of chronic cardiometabolic conditions including diabetes, hypertension, hyperlipidemia, and obesity.
  • Experience in medication adjustment.
  • Proficiency using Electronic Medical Records (EMR).
  • Experience providing care through virtual platforms including chat, phone, and video.
  • Ability to work in an interdisciplinary team environment.
  • Must be able to work 2 weekends per quarter.
  • A quiet, dedicated workspace free from distractions.
  • Reliable high-speed internet connection.

Responsibilities

  • Provide comprehensive diabetes, hypertension, hyperlipidemia, and weight loss care, education, and support for members through chat, phone, and video calls.
  • Create thorough consults aligned with internal protocols for physicians and the provider team.
  • Collaborate with Patient Care Advocates to ensure member success.
  • Develop tailored nutrition and lifestyle plans based on medical conditions and evidence-based guidelines.
  • Collaborate with pharmacists to provide guidance on chronic condition medications.
  • Track member progress, assess intervention effectiveness, and adjust plans based on clinical outcomes.
  • Provide motivational interviewing and counseling to support sustainable behavior changes.
  • Utilize member health metrics to help patients interpret lab results and track outcomes.
  • Maintain accurate member documentation in accordance with legal and internal standards.
