Requirements
- Active, unrestricted Registered Nurse (RN) license.
- Minimum of two (2) years of nursing experience.
- Hospice triage experience required.
- Must be able to work in Pacific Time Zone.
- Experience with Homecare Homebase (HCHB).
- Strong clinical assessment, triage, and critical thinking skills.
- Excellent communication and active listening abilities.
- Ability to work independently in a remote environment.
- Reliable high-speed internet and dedicated HIPAA compliant home workspace.
Responsibilities
- Answer incoming calls from patients, families, and caregivers with empathy and urgency.
- Assess pain and distress, providing clinical advice and medication adjustments under established protocols.
- Dispatch visiting nurses, social workers, aides, or chaplains for urgent in-person needs.
- Record all calls, interventions, process workflow and care plans accurately in the electronic medical record (EMR) system.
- Guide families through the steps and procedures when a patient passes away at home.
