Requirements
- Bachelor's Degree in Nursing.
- Minimum of 3 years of experience in cardiology, heart failure management, or critical care nursing.
- Strong background in heart failure patient management and Guideline-Directed Medical Therapy (GDMT).
- Current license as a Registered Professional Nurse in a compact state and California (required).
- Experience with patient phone triage in-office or via telemedicine.
- Ability to learn and proficiently use multiple EMRs and Google Workspace applications.
- Comfortable using virtual episodic vital sign monitoring devices.
- Ability to work from home in a HIPAA compliant environment.
- Advanced certification (CCRN, CHFN, RN-BC) highly preferred.
- Additional state licenses (NY, IL, FL, MO, KY, RI) preferred.
- Bilingual fluency in English and Spanish highly preferred.
Responsibilities
- Triage patients using protocols to determine care measures and when to escalate.
- Provide virtual nursing assessments to evaluate symptoms, medication adherence, and disease progression.
- Educate patients and caregivers on heart failure medications and potential side effects.
- Monitor vital signs and survey responses to identify and document changes in patient condition.
- Document all interactions and interventions accurately.
- Answer, direct, and follow up on incoming calls.
- Recognize urgent or emergent situations and initiate emergency interventions as directed by the medical provider.
