- MD or DO degree.
- Clean active license to practice in the state of New Jersey
- Compact license or multiple state license strongly preferred.
- Minimum 3-5 years of work experience following post graduate training.
- Board certification in Endocrinology.
- Must be Bilingual Haitian Creole Speaking.
- Background in family practice, internal medicine and preventive care preferred.
- Focused on preventive medicine and patient education.
- Must be proficient with computers, customer service-oriented, and able to work independently.
- Must have own laptop/tablet.
Responsibilities:
- Provide chronic disease management to patients based on our program guidelines.
- Consult with patients to understand their health concerns.
- Educate patients around healthy eating and lifestyle.
- Prescribing chronic disease management medications.
- Advise/refer patients for follow-up care.
