Requirements:

• Prior experience coordinating/navigating care for patients, caregivers, and their loved ones.
• Experience with patient populations who experience cognitive challenges is a bonus.
• Genuine empathy and patience.
• Exceptional time management and communication skills.

Responsibilities:

• Answer inbound calls, emails and text messages from patients, caregivers, families, external providers and other facilities.
• Schedule and maintain contact with patients via phone or email.
• Explain health insurance benefits and eligibility to patients.
• Follow up after all appointments to provide next steps and coordinate care.
• Under clinical supervision, document patient safety, behavior, function, medications, care needs, and advanced care plans.
• Report screening and escalate concerns to the clinical team.
• Send educational materials and promote a compassionate environment.
