Requirements:

• Must be currently enrolled and in good standing in a university’s Master of Social Work program in either Florida or New York.
• Organized, detail-oriented, and proactive.
• Deep passion for supporting members and helping them navigate access barriers.
• Excellent interpersonal and communication skills with strong inclination towards collaboration.

Responsibilities:

• Provide support to the care team by assessing and triaging social determinants of health (SDoH) needs.
• Act as a supportive partner to members in referral linkage and coordination.
• Attend weekly care team meetings and contribute as part of the Arise Care Team.
• Participate in all required trainings and meetings.
• If applicable, provide counseling services under the supervision of LCSW and attend weekly supervision meetings.
