🔍 Support services for families raising children with disabilities

  • At least three years of personal experience or two years of professional experience navigating systems for children with disabilities
  • A minimum of one year of experience providing services to families of children with disabilities or directly to children with disabilities
  • Bachelor’s or master’s-level training in social work, education, or related fields is a plus

  • Develop trust and rapport with clients
  • Work with parents to identify supports available to them
  • Determine priorities and goals for families
  • Create personalized action plans for families
  • Provide resources and guides as needed
  • Facilitate monthly 45-minute Boost calls with clients
  • Communicate regularly with clients within the app
  • Attend meetings requested by clients (IEPs, IPPs, etc.)
  • Support parents in tracking child's providers and evaluations
  • Perform administrative duties including note-taking
  • Promote and amplify Undivided's voice and culture
