- Nursing Degree is required.
- Minimum of four (4) years of WOCN or NP experience is necessary.
- At least two (2) years of wound care experience is required.
- Licensure and credentials in Colorado, with potential for multiple state licensure.
- National Provider Identifier and WOCN certification are mandatory.
- Current evidence-based knowledge of wound nursing practice.
- Experience with wound assessments and technology platforms.
- Knowledge of various debridement techniques.
- Ability to document clinical assessments thoroughly.
- Understanding of quality management and infection control.
- Collaborates with partner physicians, coordinating referrals, DME, and prescriptions.
- Assesses, examines, counsels, and determines a plan of care for wound healing.
- Orders appropriate topical products, compression therapy, and referrals based on guidelines.
- Organizes care plans through assessment, examination, and education.
- Uses remote patient monitoring to inform treatment and care management.
- Consults with home health agencies and clinicians regarding wound care.
- Participates in quality management and continuous improvement projects.
