Requirements:
Nursing Degree is required. Minimum of four (4) years of WOCN or NP experience is necessary. At least two (2) years of wound care experience is required. Licensure and credentials in Colorado, with potential for multiple state licensure. National Provider Identifier and WOCN certification are mandatory. Current evidence-based knowledge of wound nursing practice. Experience with wound assessments and technology platforms. Knowledge of various debridement techniques. Ability to document clinical assessments thoroughly. Understanding of quality management and infection control.
Responsibilities:
Collaborates with partner physicians, coordinating referrals, DME, and prescriptions. Assesses, examines, counsels, and determines a plan of care for wound healing. Orders appropriate topical products, compression therapy, and referrals based on guidelines. Organizes care plans through assessment, examination, and education. Uses remote patient monitoring to inform treatment and care management. Consults with home health agencies and clinicians regarding wound care. Participates in quality management and continuous improvement projects.
