Requirements:

• Nursing Degree is required.
• Minimum of four (4) years of WOCN or NP experience is necessary.
• At least two (2) years of wound care experience is required.
• Licensure and credentials in Colorado, with potential for multiple state licensure.
• National Provider Identifier and WOCN certification are mandatory.
• Current evidence-based knowledge of wound nursing practice.
• Experience with wound assessments and technology platforms.
• Knowledge of various debridement techniques.
• Ability to document clinical assessments thoroughly.
• Understanding of quality management and infection control.

Responsibilities:

• Collaborates with partner physicians, coordinating referrals, DME, and prescriptions.
• Assesses, examines, counsels, and determines a plan of care for wound healing.
• Orders appropriate topical products, compression therapy, and referrals based on guidelines.
• Organizes care plans through assessment, examination, and education.
• Uses remote patient monitoring to inform treatment and care management.
• Consults with home health agencies and clinicians regarding wound care.
• Participates in quality management and continuous improvement projects.
