- 3+ years of acute care hospital RN experience; 3–5-year acute oncology experience required; 1+ year of Case Management and/or Home Health experience preferred.

Requirements
- Hold an active, unrestricted RN license in your state of residence.
- Be willing to obtain additional state licenses as needed.
- Have 3+ years of acute care hospital RN experience, such as medical-surgical, post-surgical, or ICU experience.
- Have 3–5 years of acute oncology experience.
- Hold an Associate Degree in Nursing (ADN).
- A compact RN license is preferred.
- Oncology nurse or chemotherapy nurse certification is preferred.
- At least 1 year of case management and/or home health experience is preferred.
- Certified Case Manager (CCM) certification is preferred.
- Proficiency with Microsoft Office, including Word, Excel, and PowerPoint, is preferred.
- A BSN is preferred.
- Work Monday through Friday, with no weekends or holidays; occasional late-shift rotation until 8:00 PM.
- Travel less than 5% for potential clinical audits.
Responsibilities
- Conduct telephonic and occasional face-to-face assessments to evaluate members' healthcare needs.
- Develop individualized care plans to support health outcomes and wellness.
- Coordinate services, resources, and programs for members with complex medical and social needs.
- Use clinical expertise to identify risks, remove barriers to care, and facilitate transitions across the healthcare continuum.
- Partner with multidisciplinary teams to support member outcomes and effective claim management.
- Engage members using communication and motivational interviewing techniques.
