Requirements
- Hold a PharmD from an ACPE-accredited College of Pharmacy.
- Have an active Minnesota pharmacist license or be eligible for licensure.
- Hold board certification (BCGP, BCPS, or BCACP), or be willing to obtain it within 12 months.
- Demonstrate independence and sound clinical judgment.
- Maintain current knowledge of all phases of pharmacy practice.
- Have communication, documentation, and analytical skills.
- Be able to manage multiple priorities in a dynamic care environment.
- Be proficient with Google Workspace and Windows platforms.
- Bring an empathetic, patient-centered, and collaborative approach to care delivery.
Responsibilities
- Deliver disease and medication management services by phone or telehealth video.
- Conduct comprehensive medication reviews to identify therapy gaps and medication-related issues.
- Monitor drug therapy for contraindications, drug-drug or food interactions, and dosing accuracy.
- Recommend cost-effective, clinically appropriate alternatives to medical staff.
- Collaborate with internal care teams and external partners to coordinate patient care.
- Document clinical and care management activities in PharmD Live’s proprietary system.
- Educate patients on medication use, adherence strategies, and treatment goals.
- Share insights and support program enhancements for quality initiatives.
- Provide clinical oversight and support for timely client delivery.
- Present updates and progress reports to leadership as needed.
