Requirements
- Master of Science in Nursing (MSN) with specialization in cardiology or related advanced practice field.
- 3+ years of experience as a Nurse Practitioner in an outpatient cardiology or clinic setting.
- Active, current, and unrestricted Nurse Practitioner (NP) license (Compact State preferred).
- Current NP certification specific to cardiology (e.g., ANCC or AANPCB).
- Certification in Adult-Gerontology Acute Care NP or Family NP with strong cardiology focus.
- ACLS certification required or highly recommended.
- Proficiency in interpreting complex diagnostic tests such as ECGs and echocardiograms.
- Proven expertise in the practical application and protocol titration of GDMT.
- Experience with Electronic Health Records (EHR) and remote patient monitoring (RPM) tools.
- Demonstrated ability to make critical clinical decisions in complex cardiac cases.
Responsibilities
- Conduct thorough patient assessments via telehealth, including medical history reviews and physical examinations.
- Formulate differential diagnoses and develop comprehensive treatment plans for cardiac conditions.
- Manage chronic cardiovascular conditions, focusing on advanced Heart Failure, hypertension, arrhythmias, and coronary artery disease.
- Implement, titrate, and manage Guideline-Directed Medical Therapy (GDMT) aligned with ACC/AHA guidelines.
- Educate patients and families on disease prevention, lifestyle modifications, and medication adherence.
- Collaborate with a multidisciplinary team including cardiologists and PCPs to ensure coordinated care.
- Maintain accurate documentation of patient records and treatment plans.
