Requirements
- Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN).
- Current active license to practice as an RN, LVN, or LPN in the state of Michigan.
- Minimum of two years of clinical experience in Med/Surg, Case Management, or home health care.
- Hands-on experience with Electronic Medical Records (EMR).
- Proficiency with Windows desktop and Microsoft Office 365, Teams, and Excel.
- Must have high-speed internet and a Windows or Mac computer (no Chromebooks or iPads).
- Must maintain a HIPAA-compliant workspace.
- Strong initiative, judgment, organization, and time-management skills.
- Excellent verbal, written, and listening skills.
- Commitment to 20-30 hours per week of dedicated patient care.
Responsibilities
- Perform telephonic encounters with patients on behalf of partners each month.
- Develop detailed care plans within care plan templates in the electronic health record.
- Provide prevention and intervention for multiple disease conditions through motivational coaching.
- Create billable Chronic Care Management episodes to support practice revenue.
- Attend mandatory bi-monthly staff meetings and monthly one-on-ones.
