- Experience
- 1-3 years of clinical experience
Requirements
- Unrestricted RN, LPN, or LVN license in Massachusetts and a compact state
- 1-3 years of clinical experience, patient management, or disease management
- Experience performing virtual visits and telephonic care management
- Experience working with Medicare patients and diverse provider practice workflows
- Ability to work independently with minimal direction
- Strong working knowledge of Remote Patient Monitoring (RPM) preferred
- Familiarity with Chronic Care Management (CCM), Behavioral Health, or Utilization Management principles
- Ability to work in the EST time zone from 8:00am to 5:00pm
- Motivational Interviewing or Health Behavior Change experience is a plus
- Health Coach certification is a plus
- Spanish speaking proficiency is a plus
Responsibilities
- Manage physiological markers like blood pressure, weight, and blood glucose with clinical appropriateness
- Meet team goals and standards outlined metrics
- Significantly impact longitudinal patient engagement in Remote Patient Monitoring (RPM) programs
- Provide preventive health and disease management education and coaching
- Perform monthly wellness calls with assigned patients
- Lead collaborative wellness calls with patients to define health goals outlined by their care team
- Manage patient messaging and alerts
- Direct patients to treating physicians for routine questions
- Follow appropriate escalation pathways for urgent care needs
