- LCSW, LMFT, or LCPC with an active license; MFT and CPC interns considered.
- 2+ years of outpatient experience working with Medicaid or Medicare patients.
- 3+ years in assessing and managing behavioral patient care through direct clinical work.
- 3+ years with individuals having significant mental and chemical health needs.
- 1+ years of experience practicing telemedicine.
- Experience with underserved populations and socioeconomic barriers.
- Knowledge of trauma-informed and evidence-based psychotherapy.
- Comfort working in an integrated behavioral health model.
- Proficiency in electronic medical records and related technologies.
- Ability to make quick clinical assessments and manage confrontational situations.
- Effective team collaboration skills.
- Assess and diagnose behavioral health conditions, both chronic and acute.
- Formulate treatment plans for members in consultation with the health team.
- Ensure behavioral health is integrated into comprehensive care planning.
- Complete initial intakes and assess patient risks, delivering therapeutic interventions.
- Collaborate with primary care providers and other professionals.
- Provide psychoeducation to members, supporting mental health promotion.
- Support referrals for additional specialty care as needed.
- Establish trusting relationships and ensure alignment with patient goals.
- Participate in multidisciplinary case conferences.
- Provide coaching and education to team members.
- Support post-discharge planning and manage EMR documentation.
- Contribute to the design and implementation of clinical pathways.
