Requirements:

• 2-3 years in patient navigation, healthcare coordination, or medical records management.
• Familiarity with medical terminology, patient care processes, and documentation best practices.
• Strong accuracy in medical documentation and formatting.
• Ability to recognize when to escalate concerns to physicians.
• Comfortable with evolving processes and ambiguous situations.
• Experience managing patient records in EHR systems (Nice to have)
• Ability to create and maintain legally compliant care plans (Nice to have)

Responsibilities:

• Conduct initial patient assessments, recording symptoms and concerns.
• Accurately transcribe physician notes, diagnoses, and treatment plans.
• Maintain up-to-date and compliant medical records.
• Document key patient details into Haven Headache’s care plan system.
• Work with physicians to ensure accurate and thorough patient documentation.
• Assist in creating and updating personalized care plans.
• Follow up with patients post-appointment to track treatment adherence.
• Educate patients on medications, lifestyle changes, and preventive care.
• Gather patient feedback to enhance care and support services.
• Collaborate with physicians, nurses, and admin staff for seamless care.
• Handle insurance documentation, prior authorizations, and billing inquiries.
• Ensure compliance with healthcare regulations and quality standards.
• Keep patient records updated, especially after treatment changes.
• Ensure timely responses to patient inquiries and operational efficiency.
• Support insurance and billing teams in assisting patients with claims.
