Requirements:

• Fluency in English and Russian (writing, reading, speaking) is required.
• A minimum of 2 years of experience working in a healthcare setting is required.
• Knowledge and understanding of medical terminology is required.
• Knowledge and understanding of chronic care management processes is required.
• A customer service mindset for both internal and external customers is required.
• Medical Assistant and/or Medical Scribe Certification is required.
• Strong proficiency in computer software navigation; data entry and data cleansing.
• Fundamental knowledge of Google Docs, Sheets, Slides or similar.

Responsibilities:

• Provide practice support including contacting members, caregivers, and care team members as directed.
• Work closely with the clinical team to improve the health and care of members.
• Coordinate care for members of the chronic care management program.
• Conduct outreach, assessment, and service planning, and document interactions appropriately.
