Requirements:

• High School Diploma/GED required.
• 7 years of clinical, case management, or disease/condition management experience.
• RN license in Delaware is mandatory.
• Case management experience is essential.
• Preferred: Bachelor's Degree in Nursing.
• Proficiency in MS Excel and strong analytic skills.
• Excellent interpersonal and organizational skills.

Responsibilities:

• Conduct oversight over a specified panel of members with varying health statuses.
• Assess health management needs using data/analytics and clinical judgment.
• Identify appropriate clinical interventions and refer members to resources.
• Manage an active case load of members enrolled in case management.
• Conduct outreach, develop care plans, encourage behavior changes, and address barriers.
• Coordinate care and identify resources for members' health goals.
• Monitor and improve quality outcomes for the member panel.
