  • Advanced knowledge of Medicare and state Medicaid coverages of DME and supplies.

  • Experience in delivering persuasive messaging and empathy.
  • Expertise in phone etiquette and professional demeanor.

Responsibilities:
  • Arrange and coordinate prescribed laboratory tests and diagnostic procedures.
  • Support order, delivery, and receipt of DME/medical supplies as identified by the patient care team.
  • Identify and schedule referrals with external medical providers, including specialty and home health care.
  • Provide day-to-day support to the care team, including RNs, NPs, Pharmacists, and Social Workers.
  • Coordinate visits with other CareBridge interdisciplinary care team members.
  • Order diagnostics, labs, referrals, home health, and equipment and supplies accurately and timely.
  • Maintain excellent punctuality and attendance during work hours.
  • Manage large inbound and outbound inquiries, documenting all correspondences within systems.
