- Monitor patient’s condition and assess their needs to provide the best possible care and best clinical outcomes
- Observe and interpret patient’s symptoms, identify acute changes, and effectively communicate changes and or progress with physician team
- Collaborate goals of care with interdisciplinary team to devise individualized care plans for patients and families
- Provide patient education on performing routine care procedures (blood pressure measurements, administering injections etc.) and meet NorthStar Care timely patient EMR documentation standards
- Educate patients on various medications, side effects, and administration techniques
- Effectively provide treatment adjustments according to physician’s orders
- Supervise and train LPNs and nursing assistants
- Foster a supportive and compassionate environment to care for patients and their families
- Expand knowledge and capabilities by attending educational workshops, conferences etc.
- Performing other duties as assigned by the CEO and VP of Clinical Operations
