Requirements:

• Must hold a current license (LPN).
• At least 5 to a maximum of 10 years of experience.
• Experience in population health preferred.
• Proven problem-solver.
• Ability to multitask.
• Excellent communication skills, both written and spoken.
• Strong customer service skills.
• Prior use of EHR/EMR systems preferred.
• Care Management experience working with patients with Chronic Health conditions preferred.
• Bi-lingual (English/Spanish) is a plus but not required.

Responsibilities:

• Support the development of patient-centered, team-based care.
• Gather and organize patient data to identify unmet needs.
• Engage patients in their own care and enhance communication with care teams.
• Manage a caseload of 500 patients, ensuring 300 billable by month-end.
• Develop understanding of primary care practice requirements.
• Collaborate with care teams and assist in coordinating care.
• Contribute to quality improvement of population health efforts.
• Manage patient registries and support practice staff in interventions.
• Provide data to healthcare teams needed for patient care.
• Maintain courteous communication through telephone and digital interactions.
