Requirements:

• Must hold a current LPN license.
• 3 to 12 years of experience required.
• Experience in population health preferred.
• Proven problem-solving skills and ability to multitask.
• Excellent written and spoken communication skills.
• Strong customer service skills.
• Prior experience with EHR/EMR systems is preferred.
• Care management experience with chronic health conditions preferred.
• Bi-lingual (English/Spanish) is a plus.

Responsibilities:

• Develop understanding of primary care practice requirements.
• Collaborate with care teams to establish processes for patient care.
• Use Phamily platform to manage care for chronic disease patients.
• Contribute to quality improvement and care redesign.
• Manage patient registries and provide necessary data to healthcare teams.
• Support practice staff in managing target populations.
