RN Care Manager (Bilingual)

Requirements:

• Registered Nurse (RN) required with active unrestricted licensure in practicing state.
• Willingness to obtain additional state licensures as needed.
• Clear and active Nurse Licensure Compact (NLC) is required.
• Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
• 2 to 5+ years of Nephrology Nursing and/or Case Management experience preferred.
• Bilingual proficiency in Spanish required.
• Knowledge of teaching-learning process and adult education principles.
• Understanding of renal and diabetes disease processes.
• Familiarity with telehealth platforms and electronic health records (EHRs).
• Knowledge of healthcare regulations, including HIPAA.

Responsibilities:

• Collaborate with clinical and non-clinical staff and other members of the patient’s healthcare team to develop individualized interventions.
• Communicate and coordinate with the member and appropriate healthcare team members in care coordination efforts.
• Utilize motivational interviewing techniques to engage members in their care.
• Collaborate with providers to ensure successful transition from chronic kidney disease to dialysis.
• Identify and mitigate hospitalization and mortality risk for members.
• Participate in quality improvement activities.
• Evaluate program processes and make recommendations for improvements.
