Requirements:
  • Registered Nurse (RN) with active unrestricted licensure in practicing state.
  • Willingness to obtain additional state licensures as needed.
  • Clear and active Nurse Licensure Compact (NLC) required.
  • Associate Degree in Nursing (ADN) required; BSN preferred.
  • 2 to 5+ years of Nephrology Nursing and/or Case Management experience.
  • Strong knowledge of renal and diabetes disease processes.
  • Familiarity with telehealth platforms and electronic health records (EHRs).
  • Excellent communication and interpersonal skills.
Responsibilities:
  • Collaborate with clinical and non-clinical staff to develop individualized interventions for members.
  • Communicate and coordinate with members and healthcare teams during care coordination efforts.
  • Utilize motivational interviewing to encourage members' engagement in their care.
  • Ensure optimal transition from chronic kidney disease to dialysis.
  • Partner with providers to mitigate hospitalization and mortality risks.
  • Participate in quality improvement activities and project-based work.
