Responsibilities:

• Talk to, navigate, and guide members with cancer throughout their journey.
• Reach out to members by phone, text, video, and email to check in on their needs.
• Assist members in connecting with providers or resources like transportation, financial grants, emotional support, and medical equipment.
• Help members schedule health and wellness visits and conduct survey assessments to evaluate changes in their health and goals.
