Requirements:

• 3+ years of post residency work experience.
• Active state license in NJ required; CT, PA & NY state licenses preferred.
• Board-Certified in Family Medicine.
• Primary care experience and behavioral health experience, ability to treat both acute and chronic care conditions.
• Should be tech-savvy, proactive, organized, detail-oriented.
• Clean background and medical malpractice history.
• Willing to commit up to 40 hours per week, 36 being clinical, including two weekend days a month.
• Ability to speak Spanish is a plus.

Responsibilities:

• Provide text and video based comprehensive care, including acute care.
• Manage a dedicated panel of patients as their primary care physician.
• Educate patients on appropriate treatments and care plans for their health needs.
• Manage patient follow ups for chronic care programs.
• Prescribe medication and refills as appropriate.
• Drive high quality care with a focus on patient outcomes.
• Elevate remote care by providing compassionate and meaningful patient care.
