Requirements:
  • Be based in Hudson County, New Jersey.
  • Have Advanced Nurse Practitioner certification.
  • Hold current unencumbered New Jersey Licensure as a Registered Nurse.
  • Graduate with a Bachelor’s degree (BSN) from an accredited school of nursing.
  • Have an active DEA and CDS registration.
  • Obtain a Master’s degree in advanced clinical practice nursing or completion of a post Master's certificate program.
  • Have previous experience in a home care setting.
  • Demonstrate clinical competencies in age-specific care, cultural assessment and health education.
  • Enjoy working with diverse social, ethnic, and cultural backgrounds.
  • Be physically able to walk up at least 3 flights of stairs.
Responsibilities:
  • Assume primary medical responsibility for assigned patients.
  • Visit patients at home.
  • Provide person-centered care reflecting patients’ values and preferences.
  • Determine patients' prognoses and structure care plans accordingly.
  • Identify hospitalization risk drivers and minimize them.
  • Achieve high performance on quality metrics.
  • Ensure care is consistent with medical evidence.
  • Collaborate with the patient care team to address care obstacles.
  • Lead interdisciplinary team meetings.
  • Supervise and train clinical personnel on executing care plans.
  • Facilitate services from various specialties and community organizations as appropriate.
  • Provide medical administrative support.
  • Document clinical encounters accurately and timely.
  • Engage in professional development.
  • Collaborate in continuous quality improvement initiatives.
