Requirements:

• Doctor of Pharmacy (PharmD) designation is required.
• Must be a licensed Pharmacist in the state of Montana.
• Minimum of 1+ years of experience as a clinical pharmacist, with completion of a PGY1 residency.
• Completion of PGY2 residency specializing in oncology is preferred.
• Strong verbal communication skills and in-depth knowledge of pharmacology and FDA regulations.
• Proficient in using relevant database applications and Rx30 software system.
• Ability to analyze patient information and exercise good judgment in clinical decisions.

Responsibilities:

• Advancing quality pharmacotherapy care for Oncology patients through a hands-on approach.
• Collaborating with team members in an outpatient oncology practice.
• Providing face-to-face and phone-based disease management to patients.
• Reviewing prior authorization requests for medications.
• Documenting pharmacotherapy outcomes and monitoring patient medication therapies.
• Consulting with patients to resolve medication-related problems.
• Participating in process improvement initiatives.
