Requirements:

• Based in Hudson County, NJ, or neighboring areas.
• MD/DO degree and Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatric Medicine, and/or Palliative and Hospice Medicine.
• Active license to practice in the state of New Jersey.
• Active CDS and DEA.
• Preferred experience in a house calls setting, fee-for-service and/or value-based.
• Enjoyment of working with diverse patient backgrounds.

Responsibilities:

• Be accountable for the care of a cohort of patients, in collaboration with nurse practitioners.
• Coach Nurse Practitioners to ensure clinical excellence.
• Ensure patients can stay at home when possible.
• Visit patients in their homes and through telehealth.
• Participate in Interdisciplinary Team Meetings, contributing to care plans.
• Achieve high performance on quality metrics.
• Provide care consistent with medical evidence.
• Involve other providers in the care team.
• Facilitate consultations and additional care as needed.
• Complete accurate documentation of clinical encounters.
• Collaborate with Medical Director and Practice Manager on quality improvement.
• Participate in on-call rotations.
• Promote a positive image for the practice.
