Requirements:

• Certification as an Advanced Nurse Practitioner (required).
• Current unencumbered NJ Licensure as a Registered Nurse.
• Bachelor’s degree (BSN) from an accredited nursing school.
• Active DEA and CDS registration.
• Master’s degree in advanced clinical practice nursing or graduate of post Master’s certificate program for Nurse Practitioner.
• Experience working in a home care setting.
• Demonstrated clinical competencies in age-specific care, cultural assessment, and health education.
• Experience working with diverse social, ethnic, and cultural backgrounds.
• Comfortable walking up at least 3 flights of stairs.

Responsibilities:

• Assume primary medical responsibility for assigned patients.
• Visit patients at home to provide care.
• Provide person-centered care reflecting patients' values and preferences.
• Determine patients' prognoses and structure care plans accordingly.
• Identify drivers of hospitalization risk and address them.
• Achieve high performance on quality metrics.
• Ensure care is consistent with medical evidence and protocols.
• Identify obstacles to care and collaborate to resolve them.
• Lead interdisciplinary team meetings.
• Supervise execution of care plans by clinical personnel.
• Facilitate consultations with specialists as needed.
• Document all clinical encounters accurately and timely.
• Participate in professional development and quality improvement efforts.
