- A Board-Certified Nurse Practitioner or Physician Assistant.
- Minimum of 2 years of specialized cardiology experience in either ambulatory or inpatient settings.
- Current active and unencumbered license to practice in California, Florida, and New York.
- Ability to work from 11am to 7pm EST.
- Previous telehealth experience is an advantage.
- Motivated to provide health education and grounded in health promotion techniques.
- Ability to collaborate with healthcare providers and non-clinical personnel.
- Proficiency with Mac, Windows-based applications, email, and web skills.
- Ability to learn and navigate multiple EMRs.
- Must work from home in a HIPAA-compliant environment.
- Virtually evaluate patients and assist in creating care plans based on their needs.
- Perform virtual video visits and assess both acute and non-acute clinical issues.
- Document assessments in EMR according to guidelines and available patient data.
- Respond to alerts about patient vital changes and symptoms in a timely manner.
- Independently manage clinical changes within scope and escalate as needed.
- Liaise with virtual care team for proper medication orders and patient communication.
- Serve as the point of contact for patients' primary care teams.
- Educate patients on healthy behaviors and self-management.
