Requirements:

• 2+ years of work experience in eye care.
• Must be a Certified Ophthalmic Technician (COA, COT, COMT).
• Proficient with Electronic Medical Record (EMR) systems, preferred.
• Strong communication skills, both written and verbal.
• Excellent organizational skills and customer service.
• Ability to work in a team with minimal supervision.
• Reliable WIFI access and HIPAA-compliant home office.

Responsibilities:

• Communication and engagement with patients, families, and healthcare providers regarding care.
• Coordination with home health agencies and community services.
• Collection of health outcomes data and documentation.
• Patient and family education for self-management and daily living.
• Assessment of treatment adherence and medication management.
• Identification of community and health resources.
• Facilitating access to necessary care and services.
• Ongoing review of patient status and care plan development.
