Requirements:

• High School Diploma/GED with RN license or Master's Degree if licensed Behavioral Health Professional.
• Preferred: RN with BSN or MSN.
• 3 years of post-licensure experience in behavioral health clinical, case management or condition management.
• Preferred: training in cognitive behavioral therapy (CBT), experience with diverse populations, proficiency in a second language.
• Required licenses: RN, LSW, LCSW, LMSW, LPC, LMHC, LMFT, or PhD.

Responsibilities:

• Function as a primary contact for members with behavioral health needs.
• Conduct clinical and psycho-social assessments through telephonic interactions.
• Develop case-specific care plans and maintain regular contact with members.
• Communicate with treating providers in complex situations.
• Identify and assist members in accessing resources for their health goals.
• Incorporate lifestyle improvement opportunities into member interactions.
• Document activities in compliance with business and regulatory standards.
• Collaborate with management teams to provide co-management activities.
• Participate in training related to behavioral health.
