Requirements:

• Reliable transportation with the ability to travel throughout Hartford, CT.
• Previous experience in community outreach, event planning or sales.
• Proven track record of developing leads and meeting set targets.
• Effective written and verbal communication skills with diverse audiences.
• A passion for providing high-level customer service with empathy and active listening.
• Ability to articulate case statuses through detailed notes on Salesforce.
• Computer proficiency with strong typing skills and problem-solving capabilities.

Responsibilities:

• Proactively create and nurture new relationships within communities.
• Assist in organizing and hosting events to raise awareness for CDPAP.
• Build and maintain partnerships with local organizations and healthcare providers.
• Develop and implement tactics to reach target demographics.
• Independently create reports to track lead development.
• Collaborate with internal teams on outreach efforts.
• Generate qualified leads for the CDPAP program, aiming for 10-15 per week.
• Track and report on outreach activities and partnerships.
• Act as a primary representative for the company.
