Requirements:

• Maintain a minimum of 5 licenses in states where Theoria actively provides care.
• Be prepared to apply for and obtain new licenses as requested by Theoria.
• Reliable high-speed Wi-Fi is required.
• Timeliness in attending scheduled shifts is imperative.
• Experience with EHR systems like PointClickCare, MatrixCare, and American HealthTech is beneficial.

Responsibilities:

• Manage the clinical care of the population at assigned skilled nursing facilities (SNFs).
• Monitor message boards, answer calls, and communicate with assigned SNFs during shifts.
• Provide high-quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
• Timely documentation of all patient care encounters is required.
• Available for phone/secure messaging consultations with facility nursing staff.
• Complete provider documentation in Theoria’s proprietary electronic health record (EHR) system, ChartEasy™.
• Attend remote scheduled shifts punctually.
