Requirements:
  • At least 1 year of previous experience in a fast-paced customer service role.
  • Previous experience managing multiple schedules.
  • Strong attention to detail.
  • Exceptional written and verbal communication skills.
  • Ability to multitask.
  • Excellent organizational and time management skills.
  • Strong critical thinking and problem-solving skills.
  • Proficient in Microsoft (i.e., Word, Excel, Outlook).

Responsibilities:
  • Conduct orientations involving signing of paperwork.
  • Perform initial assessments and evaluate the home care needs of the client during orientations.
  • Gain an understanding of the client’s daily living environment.
  • Identify opportunities to support the caregiver to provide proper care.
  • Train caregivers on use of the FreedomCare App for submission of Daily Notes.
  • Interact with the Health Coach, as needed.
  • Engage in interdisciplinary meetings with care managers, caregivers, and patients.
  • Collaborate in care and communicate impressions of cases to appropriate staff.
  • Critically assess procedures and identify improvements for evaluation of cases, feedback to appropriate parties, and training of caregivers.
  • Wear multiple hats as operations develop.
  • Other duties as assigned.
