Requirements:

• 1+ years of general work experience, preferably in case management.
• Must be located in California.
• Field Ops require reliable transportation for community engagements.
• Virtual Ops require a quiet, HIPAA-compliant workspace.
• Bilingual in English and Spanish.
• Strong understanding of cultural fluency.
• Empathy and collaborative team skills are essential.
• Organizational and technical skills including CRM databases.

Responsibilities:

• Maintain ongoing caseload of individuals using evidence-based approaches to achieve health goals.
• Support members with social navigation using relationship-based strategies.
• Conduct periodic outreach via telephonic and SMS for timely follow-ups.
• Work with members to identify health/wellness goals and incorporate these into care plans.
• Collaborate with the care management team and ensure compliance with health plan regulations.
